He's irritated enough about the refusal to answer to not want to accept any help, but he doesn't like the idea of Komaeda doing his dirty work either. It's only worthless, pathetic men who need another to do away with their demons.

He wouldn't be surprised if the real goal was to experience Hiyori's talent first hand. But he's struggling to come up with anything better when his condition is so poor, and he knows the right way to make an i incision without doing permanent harm.
